Pulmonary Rehabilitation Program

Living with a chronic lung condition can make everyday activities more challenging. Our 8-week Pulmonary Rehabilitation Program is designed to help individuals with COPD and other chronic pulmonary conditions improve their physical activity, manage symptoms, and build confidence in managing their lung health.
The program combines supervised exercise, practical education, and personalized support from a multidisciplinary healthcare team. Each session is designed to help participants safely develop the skills and strategies they need to stay active and maintain a healthy lifestyle.
What Does the Program Include?
Participants attend two sessions per week. Each session includes supervised exercise tailored to individual abilities, along with education on important aspects of living with chronic lung disease.

Exercise and physical activity

Medications and medication management

Nutrition and healthy eating

Sleep and fatigue management

Stress management
-
Infection prevention

Environmental factors and lung health
-
Smoking cessation

Healthy lifestyle strategies
Our multidisciplinary team provides evidence-based education, guidance, and support to help participants better understand their condition, manage symptoms, improve their physical activity, and incorporate healthy habits into their daily lives.
Who Can Participate?
The program is available to individuals with a diagnosed pulmonary condition who may benefit from pulmonary rehabilitation.
Participants must be referred by a healthcare professional, including:

Physician

Nurse Practitioner
-
Physiotherapist
-
Respiratory Therapist
All referred patients will be assessed by a physician with expertise in pulmonary rehabilitation to determine program suitability and provide recommendations.
Required Referral Documentation
Please ensure the following documentation is included with the referral:

Completed referral form
-
Recent pulmonary function testing, preferably completed within the last 6 months
Spirometry is accepted if full pulmonary function testing is unavailable. -
Current medication list, including dosages
