Pharmacist
The Family Health Team pharmacist collaborates with all the members of the team to help optimize patient’s drug therapy by identifying, preventing and resolving drug related problems. They provide drug information to patients and members of the team through individual counselling or group teaching. Patients can meet with the pharmacist one on one for a medication review and to discuss how their medications work, how much and how often to take it, possible side effects and how to manage them, and suggestions on how patients can adhere to their medication schedules. The pharmacist will help patients set goals for self management of their medication routine while consulting with the physician, other health professionals, and the community pharmacist. Heather’s specialties include asthma, diabetes, COPD, wellness and fitness(lifestyle coaching), and smoking cessation.

Family Practice Nurse (FPN) advocates for and supports patients as they navigate through the health care system. FPN offer health promotion, health education and disease prevention care and education (including disease specific teaching, medication review and management, lifestyle counselling and immunization information). FPN works collaboratively with physicians, nurse practitioners and other members of the allied health team assisting them with several aspects of patient care. FPN performs clinical skills such as administering immunizations, allergy injections, ear flushing, suture removal and dressing changes. FPN are also involved in well-baby and family care that ensures continuity of care from birth to end of life.
Smoking Cessation Specialist – specializes in helping people with their smoking cessation efforts. A Smoking Cessation Specialist has specialized education around smoking and smoking cessation.

Tanya Cellucci’s approach to therapy is heavily driven by the concepts behind Cognitive Behavior Therapy (CBT). CBT focuses on reframing negative thoughts into more positive thought patterns with the goal of improving mood, and relieving anxiety and/or stress. With CBT, we not only work on reframing the thoughts, we also look at engaging in productive behaviors that can help an individual overcome difficulty in their lives. CBT is considered one of the most effective approaches for many mental illnesses, specifically anxiety and depression. Tanya’s approach is very goal-oriented, as she will teach you and practice with you strategies and skills to help you to overcome your current difficulties or struggles, whatever they may be, as well as be prepared to manage and overcome any future obstacles you may encounter in your life. In addition to CBT, Tanya uses other therapies inclusive of; Dialectical Behavior Therapy, Mindfulness-Based Cognitive therapy, Interpersonal therapies, Positive Psychology, Solution focused-brief therapies, and many other approaches.

Mental Health Counsellors provide patients with support and understanding in regard to their particular situations. Whether its anxiety, depression, stress management, or processing, the Mental Health Counsellor helps patients expand their perspectives through a therapeutic dialogue. In addition to talk therapy, effective coping strategies and personalized self-management techniques are discussed. The overall goal of the therapeutic relationship is to help provide patients with the resources and strategies to effectively manage, handle, and process their situations. Greg Turk specializes in Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, and Narrative Therapy. He embraces the importance of ongoing self-care and puts his focus on improving circumstances in the present so the patient will have the resources needed for the future.

Collaborates with the interdisciplinary team to transform their insights into actionable programs and services. The program coordinator also promotes awareness of the WFHT services to the FHT patients, physician partners and the community. You may also see me jump back into my role as an RN at the clinic as the need arises.

Certified Tobacco Educator – specializes in helping people with their nicotine dependence quit efforts. A Smoking Cessation Specialist has specialized education around tobacco and nicotine dependence and cessation.
