Woodbine Family Health Team is a collaborative, multidisciplinary organization of health care professionals dedicated to providing comprehensive, patient-centred care.

Our services are available to patients who are rostered/registered with one of our physicians, allowing our team to support a wide range of health needs through coordinated and integrated care.

Our Clinical Team:

Family Physician

Family Physicians provide a comprehensive assessments of patients inculding prevention, diagnosis and treatment of illnesses or injuries, as well as chronic disease management.

Nurse Practitioner

Nurse Practitioners provide comprehensive assessments of patients: prevent, diagnose and treat illnesses or injuries, and help patients manage their chronic illnesses.

Registered Nurse

The Registered Nurses (RN) provide preventative health examinations, conduct health education, and monitor patients with chronic diseases. Our RN is also a Reigstered Diabetes Educator who specializes in Diabetes prevention and management.

Registered Dietician

Dieticians provide advice on how nutrition and diet impact on maintaining health, as well as helping with dietary measures for prevention and treatment of diabetes, heart disease and other conditions.

Mental Health Counsellor

Mental Health Counsellors help families by providing counseling /therapy or by finding and referring to community resources where and when needed.

Respiratory Therapist

A Respiratory Therapist (RT) is a regulated health care professional who specializes in the assessment, treatment, and management of breathing and lung conditions. Respiratory therapists work with patients of all ages who have difficulty breathing or conditions affecting the heart and lungs.
