CHRONIC DISEASE SELF MANAGEMENT PROGRAM

Living a Healthy Life with Chronic Health Conditions

Do You Have an ongoing health condition such as heart disease, diabetes, chronic pain or lung disease?

Program Details

2 hours a week for 6 weeks
Developed by the Self-Management Resource Center
Led by two trained Peer Leaders either in-person or virtually through Zoom
Receive the “Living a Healthy Life with Chronic Conditions” resource book
No referral needed – just sign up!

Workshop Topics

Healthy eating  & Starting and maintaining physical activity
Medication management
Working with your healthcare provider(s)
Action planning & Problem solving
Dealing with difficult emotions

Each session builds upon the last – attendance to each session is highly encouraged,

You will be free to share and experience how many other people live with similar issues. The group will get to work together, you will get great information to refer to from the textbook.

Developed by the South West Self Management Program, this workshop focuses on what to do before, during and after a healthcare appointment through action planning. Ultimately, patients are the most important members of their healthcare team.
